Home Health Care is medical care prescribed by your physician, as part of your care plan following a hospitalization, illness, or progressing debility. The Home Health Care Team can include an RN, Physical Therapist, Occupational Therapist, and other skilled professionals experienced in helping you transition back to independence safely from home. Home Health is covered by Medicare and other insurance.

We believe people who need home health care should get the very best. Our goal is to provide you with quality personalized care through coordination of skilled care and services. Under the direction of your physician, our team of qualified health care professionals will provide on-going case management as needed.
Our Home Health Care Services Include:
Nursing visits will consist of a complete physical assessment and evaluation of the patient’s condition. The skilled nurse will also instruct the patient and family/caregiver in disease process, medications, medications management, and other pertinent topics.
Physical Therapists provide skilled therapy services including assessment of your strength, balance, coordination, endurance, mobility, and pain. The physical therapist develops an exercise program to help you regain movement, strength, and endurance. They will also help you with safety in your home.
Occupational Therapists provide skilled care that includes assessment of your ability to perform activities of daily living such as bathing, dressing and household tasks. The occupational therapist will develop an exercise program to strengthen arms and hands and improve coordination. They will teach you how to safely perform self-care activities to enhance your independence.
Speech Therapists provide a skilled assessment of speech language and swallowing problems. They provide therapeutic services to help you communicate better and teach methods to improve cognitive impairments. They will teach methods to improve memory, reading and understanding.
Home Health Aide (HHA) visits may be assigned by the RN or PT following evaluation of your needs. HHAs will provide personal care (bathing) and related services for the patient and other care as appropriate. HHAs cannot administer medications.

What to expect after your surgery
- Aventas Home Health will contact the hospital care team in preparation for your return home. We will follow your progress and be ready to start care when you are discharged.
- Home Health Care brings the RN and Therapists to you at home – to help you properly heal in the first few weeks after surgery – we will evaluate your home for safety, make recommendations, and begin therapy for optimal rehabilitation.
- An experienced therapist is crucial to keep you safe at home and develop the right plan that follows your Doctor’s protocols and precautions, and includes your individual needs and challenges. Aventas Therapists average over 20 years’ experience. Our trained and knowledgeable Therapists will work with you one on one to reach your rehab goals.
- Aventas will follow your Doctor’s orders. We will communicate with your Physician about your progress, any concerns, and answer all your questions.
- Aventas will call you before your surgery and answer any questions you may have.
